C.I.S.D. Immunization Requirements
Dear Parent,
Texas state law requires that a student’s immunization record be current for the child to attend school. Our records indicate that your child is lacking the following HIGHLIGHTED immunizations. Until compliant records are received in the clinic office your child will not be enrolled or able to attend school.
DPT (diphtheria, pertusus, tetanus
5 doses are required; only 4 doses are required if at least one dose was received on or after the 4th birthday.
*Beginning August 1, 2009, all students entering the 7th grade will be required to have a Tdap booster five years since their last dose of a tetanus-containing vaccine.
IPV (polio)
4 doses are required; only 3 doses are required if at least one dose was received on or after the 4th birthday.
MMR(measles, mumps, rubella)
*2 doses are required on or after the first birthday on all students entering kindergarten.
HIB(haemophilus influenza)
Must have begun the series.
HBV (hepatitis B)
3 doses are required.
HAV (hepatitis A)
2 doses given on or after 1 year of age are required on all students entering PK and kindergarten.
Varicella(chicken pox)
*Two doses are required on or after the first birthday on all students entering kindergarten and 7th grade. A written statement attesting to the child’s positive history of the disease is acceptable in lieu of either dose of the vaccine.
Prevnar/PCV7(pneumococcal vaccine)
Students enrolled in a pre-kindergarten program must have begun the series.
Menactra/MCV4(meningococcal vaccine)
*All students entering 7th grade will be required to have one dose.
*Change in Texas Immunization Requirement effective August 1, 2009
- School has no record of your child’s immunizations
- Child has a summer birthday and is due as soon as possible after DOB__________.
- A current immunization record is due now.
